Your Health Navigator will work with you to design a personalized plan

The Navigation team is comprised of a physician advisor and registered nurses who provide care coordination for seniors, individuals with disabilities or medical challenges, and their families. 

Care Coordination is client centered with a holistic approach. The process begins with a comprehensive assessment performed in the individual’s residence. The design of a service plan focuses on articulating the client’s goals related to how they want to live and identifies short term and long-term interventions.

Bonnie Castonguay, RN, Chief Executive Officer is a quality improvement and health outcomes expert with more than 35 years of experience in nursing, hospital quality, and care management. Prior to founding Ho’okele Health Innovations and Vivia, Bonnie served as the Vice President of Quality at Hawaii Pacific Health. Bonnie is an innovator in the field of complex care management, leading a team of nurses under a federal grant that helped the most complex patients navigate care in their own homes and saved the payer more than $1M in less than a year. With more than 15 years of experience leading care navigation and home operations, Bonnie is passionate about coordinated care for seniors which includes well-trained caregivers in the field.

Mary Lester, RN, is a rural community leader in home care on the island of Maui. Mary was the start up executive who opened Ho’okele’s Maui operations from ground zero. Prior to joining Ho’okele, Mary was the Executive Director for Home Care for Hale Makua Health Services. Navigating the lower income demographic of the rural community, Mary has become expert at government contracted services and champions the cause of accessibility.

ANGELYN ENRIQUEZ RN

Angelyn Enriquez was born and raised on Oahu. She is an alumnus of Sacred Hearts Academy.  She graduated from the University of San Francisco where she received her Bachelor of Science in Nursing. Angie has more than 20 years of nursing experience in the acute care setting. She started her nursing career at St. Francis Medical Center where she worked as an oncology, transplant, and ICU nurse. She currently works in the recovery room at Queen’s Medical Center and in the ICU at Pali Momi Medical Center. She has been with Hookele Health since 2021. Angie enjoys spending time with her family and friends, hiking, photography, trying new foods, and traveling.

DR. ARNOLD KOP

PHYSICIAN ADVISOR

Dr. Arnold Kop was born and raised in Honolulu, Hawaii. He received his medical degree from the University of Hawaii John A. Burns School of Medicine. He became board certified after completing a residency in Internal Medicine at Kaiser Santa Clara in California.

Dr. Kop returned to Hawaii to establish the Hospitalist program at Kaiser Moanalua Hospital. He eventually served as chief of the Hospitalist program and remained with the department for 17 years.

He transitioned to Kaiser’s Department of Continuing Care for the next 11 years, serving as Home Health Director and providing care to the geriatric population in nursing homes throughout the state.

Dr. Kop has been active in the Boy Scouts of America and has served on the Board of Trustees at St. Louis School. His hobbies include watching football games for all his children’s universities, including the University of Utah Utes, the United States Air Force Academy Falcons, and the Utah State University Aggies.
