The Navigation team is comprised of a physician advisor and registered nurses who provide care coordination for seniors, individuals with disabilities or medical challenges, and their families.
Care Coordination is client centered with a holistic approach. The process begins with a comprehensive assessment performed in the individual’s residence. The design of a service plan focuses on articulating the client’s goals related to how they want to live and identifies short term and long-term interventions.
“Ho’okele was excellent. When my mom received the diagnosis of ALS, we were all devastated. Our health navigator helped keep us focused and gave us options to choose from. The most important thing for me was that she was able to give us the emotional support that we needed.”
